Budget impact analysis of a flash monitoring system for glucose control in patients with type 1 diabetes mellitus in the perspective of the public health system in Brazil

Abstract

Objective: This study aims to investigate the budget impact with the introduction of a flash glucose monitoring system for glucose measurement in people with diabetes mellitus type 1 in the perspective of the public health system (SUS, in Portuguese). Methods: The model was built to investigate the diferences in costs comparing the current scenario (monitoring with glucometer and tests strips) to the new scenario including the adoption of a flash glucose monitoring system for diabetic patients with greater risk of presenting hypoglycemic events in a time horizon of five years. For costs calculations, it was considered the expenses with the acquisition of technologies, hospitalizations by hypoglycemia and ketoacidosis. Data for the clinical effects were obtained from observational studies and clinical trials published in the literature. Results: The budget impact analysis for the introduction of a flash glucose monitoring system for people with diabetes mellitus type 1 in the perspective of SUS estimated an average incremental cost of R$ 651,87/ patient/year. In five years, the adoption of a flash glucose monitoring system will have an incremental budget impact of approximately R$ 4,8 billion, representing an increase of 73.7% in the expenses with diabetes mellitus type 1 in the fifth year after the incorporation. The sensitivity analysis indicated that the model was more sensitive to variations in the cost of the flash glucose monitoring system and in the eligible population. Conclusions: The incorporation of the technology will have an import impact on the expenses for the monitoring of glycemia in this population. Therefore, the implementation will depend on the definition of restrictive criteria to access the device and/or price negotiation between the Brazilian Ministry of Health and the manufacturers/suppliers of the technology.
